The Permission, Release, and Emergency Contact Form must be completed by the parent/legal guardian of all participants in advance of each workshop for which your girl is registered.

The Media Release Form must be completed by the parent/legal guardian of all participants in advance of the start of workshop.  This is a one-time form that will apply to all workshops.
